I got this jammed in a GM parts manual, stuck in the pages.  Never seen or heard of the dealer (USA) or the model of car....

Looks like it came from Gaffney, SC - birthplace of Andie McDowell, represented by Kevin Spacey in House of Cards, and home of Freightliner Custom Chassis.
