I called George recently and he informed me that all requests must be faxed into them. He said that people would request the information and when nothing was found they did not feel they should have to pay. There is a minimal fee now for all requests as of July 18, 2008. He emailed me the form and will do the same for anyone but it has to be faxed in to them.
