There were a number of conversions done before they came out with factory versions. that looks like a really well done one. i had a chance to buy an old forestry 1968 chev that used the three door suburban as a base so it was a three door crew cab. I didn't do it and a sorry to this day as it got trashed shortly thereafter.
'64 Parisienne CS "barn find" - last on the road in '86 ... Owner Protection Plan booklet, original paint, original near-mint aqua interior, original aqua GM floor mats, original 283, factory posi, and original rust.
I have also seen a 1959 version of that truck. It was a 3/4 ton Apache 36 Stepside. I saw it on Mount Pleasant Road in Toronto back in 1981. It was wearing the factory deep green paint and was in really nice shape.
A few years later I had seen in an Auto Trader a 1960 version on a 1-ton chassis cab with dual rear wheels and a ramp (old style car hauler).

1966 special factory order 2dr. crewcab. older 350 engine with low km. 5-speed from 96 chevrolet truck. 373 rear gear. tight front end.interior freshened up with new upholstery and carpet. rallye wheels
The astute would of course recognize that that truck has a 1965 cowl emblem, not fender mounted 66 emblem (which is different from a 1964 fender emblem).
