From my 65 Pontiac. Base is stamped GUIDE 3, SAE L64. Lens is GUIDE 5 SAE L 65, 5956698 1.
I'd say mine is like your silver one with the ears modified. I'm also assuming your lens of a different part number is different than mine, in that it has no silver paint inside 1/2 the lens. I'm assuming mine is original to the car.
Well it looks like I need the ebay repro lamp since it looks like what I have now if mine hadn't been repair modified.
The trunk harness connector hasn't been cut and replaced either which is further evidence.
My car is a Dec. 64 build so it predates your SAE L 65 lens, but other than that my lamp appears the same.
My US Pontiac parts book only goes up to 66 but it shows 65-66 US models use the same lens. All 65 US models use it except wagons, that includes Lemans, all 66 US all models including Lemans use it.

Thanks for all the replies. I've looked at the numbers in the Dec. 64 Canadian Chevrolet catalogue wiki pages 7829 and 7830 here: www.gmpartswiki.com/getpage They say 5956396 for the lens number which differs from my 5955195 lens number, but the lamp housing number is only found on it's original box though. My 5955195 lens is for 66-69 all as per the catalogue page posted by cdnpont. I don't understand how the 66-69 lens could be SAE 64 coded if it supercedes the SAE 65 coded 5956396 lens specified in the Dec. 64 Canadian Chevrolet catalogue?
